Online Meet Entry Form

This form will allow you to enter all the necessary information to enter any Team Delaware Coached meet and will also give you the option to print a confirmation page for your records. After completing the form click the submit button and the information will be sent via email directly to Dan Sullivan.

Submit one swimmer per meet per form.  Complete the 11 step form below
After selecting from drop down boxes do not use the mouse wheel to navigate as it will change your drop down box selections.  Use arrow down buttons to move around the form.

 # 3  Are you a lessons swimmer entering the meet:  

 # 4 Birth Date: (MM/DD/YY)         

 # 5 Swimmers Age as of first day of meet:

 # 6 List the Event Numbers and Event Name you want to swim and any comments you wish to aid in the entry process

Team Delaware Volunteers:  When Team Delaware hosts a meet it is the teams responsibility to supply meet workers for all the necessary jobs to run the meet, your entry will NOT be valid unless the space below is completed with your parent volunteer selection
